Many individuals have asked ACA to provide address information on the mailing of Proof of Life forms for Social Security. We have the following information to share from the Social Security Administration.

Depending on the Proof of Life form beneficiaries receive, the beneficiaries will send the forms back to the P.O. Box that is listed on their return envelope:
